Possible hazards observed:
It was noted immmediately that the working environment did not contain sufficient lighting, an immediate hazard when working in proximity to sheer drops. Although safety railings have been erected in some areas, this was not considered to be sufficient when considering that walkways were often hazardous (environment is very damp and full of bat droppings, which proved slippery underfoot).
Recommendation: Risk assessments to be carried out including risk of possible infection from previously-mentioned bat dung.

Possible hazards observed:
Despite obvious efforts on the part of the employer to achieve a level of health and safety awareness in this workplace, several omissions were noted. These included lack of staff training (workers were noted standing in very close proximity to heavy lifting equipment) and improper storage of heavy munitions.
Recommendation: Schedule continued risk assessments at regular intervals, particularly in regard to lack of emergency automatic repressurisation mechanisms in place around airlocks.

Possible hazards observed:
As is typical with this sort of "lair" used for intense physical and combat training, personnel safety is often left to procedure and personal responsibility, rather than through proper warning signs and markings. Some obvious issues of note were the poor lighting (composed almost entirely of candles, some of which were regularly and deliberately extinguished as part of the normal working day) and a particularly complicated network of training ropes that posed a significant trip hazard.
Recommendation: Schedule risk assessments, move stable facilities elsewhere or put in ventilation to improve working environment.

- Type of environment: Military facility
- Number of employees: 1,205,309 personnel and 400,000 droids
- Risk assessments completed? yes
Possible hazards observed:
It was determined that the length of this visit was not sufficient to fully inspect the entirety of the facility. Despite this, a number of initial hazards were identified. These included lack of warning signs and inadequate safety railings near cavernous drops, possible slip hazards from overly-polished floors, possible trip hazards at blast doors, and risk of ankle injury from small, apparently out-of-control wheeled droids. Furthermore, there seemed to be little in the way of emergency exits, leading this inspector to believe that in the event of a catastrophic accident, few employees would be able to safely escape.
Recommendation: Schedule further visits. Bring more inspectors.

- Type of environment: Lair of mad Irish bomber

Possible hazards observed:
Use of previously abandoned facilities often requires significant investment to make the working environment suitable for employees to inhabit. Sadly, no evidence of such effort was found during this inspection. Despite the single employee's insistence that he did not mind, the inspection yielded unacceptable levels of filth and decay for a workplace. Additionally, the ship's angle in the water meant that all of the floors were tilted inappropriately, leading to risk of injuries called by falls. Furthermore, several booby traps appeared to have been set up, with the apparent deliberate intent to cause injury from explosive devices and sharp objects. The inspection was abandoned at this point as there seemed little need to continue.
Recommendation: Close facility immediately and instigate prosecution against employer for completely unacceptable working conditions.
